‎For practitioners updating or adding their report delivery information for laboratories within the Interior Health Authority,  

Please complete the Physician Activation e-Form and submit it directly through the link located on the bottom of the form. 

Please complete the Healthcare Provider Information Change form (PDF) and send it via fax to 1-877-412-4440 or local 604-412-4445; or you may print and forward it via your local LifeLabs courier.

Please ensure you are first authenticated in NH Cerner to receive any results or reports. 

Then, complete the  CDX Activation Form for the Clinic to which they are receiving results, as well as the Information Sharing Agreement (ISA)

Submit each document directly through the links located on the bottom of each form respectively.

For fax delivery of reports,  please also send the completed forms via fax to 604-291-6837 in order to validate the fax number for the delivery of reports.
