Pharmacists must register with laboratory operators, providing test result distribution and contact information, MSP practitioner number and other details, prior to ordering lab tests.

Please complete the Pharmacy Registration Form (PDF) and send it via fax to 1-877-412-4440 or local at 604-412-4445.

Pharmacists will be notified when registration is completed.
