Advance Care Planning

Advance care planning is the process of thinking about and sharing your values, beliefs, and preferences towards care with your loved ones and healthcare team. It is important to have an advance care plan in place, regardless of your age or stage of health. 

Having an advance care plan:
  • Ensures your wishes for your future health care are understood
  • Allows for these wishes to be met in the case that you become unable to speak for yourself or make a decision about your care
  • Supports care that is aligned with your goals, values, beliefs, and preferences
  • Improves quality of life
  • Reduces anxiety and depression in times of crisis or at end of life
  • Improves satisfaction with your care
