How to Request Access to CareConnect for Community-Based Users

This page is intended for community-based healthcare providers who would like to request access to CareConnect.
Community-based healthcare providers and eligible team members may request access to CareConnect solely to support the delivery of direct patient care. Access for secondary use purposes such as research, quality improvement or quality assurance is not permitted. 

To obtain CareConnect access, you will need to complete the steps outlined below. This entire process will take between 30 minutes to 1 hour to complete.

Register Your Worksite
If your worksite has not been registered yet, please appoint someone from your worksite to be your Group / Site Administrator (typically a lead MOA, office manager or physician).

Note: Before a worksite is approved for creation, an individual with signing authority for your worksite must complete the Worksite Privacy & Security Declaration form.

You will receive an email which contains instructions on how to complete this requirement once the worksite creation request has been submitted.

Join a Worksite and Enrol as a User
Once a worksite has been created, an individual user must request to 'join' every worksite that they intend to access CareConnect from. Every eligible CareConnect user must register their own unique account in order to access patient information via the CareConnect eHealth Viewer. 

Refer to the following role-specific Quick Start Guides for an overview of the registration process. Additionally, you can refer to the linked How-To Guides that contain detailed step-by-step instructions and screenshots. 

These practitioners can request access to CareConnect directly. They may also authorize On-Behalf-Of users to view CareConnect with the same permissions as they have.

Using CareConnect to Support Direct Patient Care
Once the above requirements are complete, you will receive a Welcome Package via email with instructions on how to access CareConnect to view patient records and support direct patient care. 

Browser Requirements for My.CareConnect and CareConnect

Microsoft Edge is the browser officially supported by Community Based Access to CareConnect. While CareConnect does work on other browsers, your experience may not be as seamless as Microsoft Edge.
