Advance Care Planning

Advance care planning is the process for people to think about and share their values, beliefs, and preferences towards care with their loved ones and healthcare team. 

Clinicians have an important role in facilitating discussions with patients around advance care planning, to ensure their wishes for future health care are understood and put in place. 

PHSA provides a variety of services related to advance care planning:
  • Training, including serious illness conversations for clinicians
  • Consultation on advance care planning-related issues
