This project helps physicians generate a point-in-time health record summary using standardized information about a patient’s health, treatment and demographic information, making it easier for care providers to make informed decisions and coordinate transitions in care.
Care providers will be able to generate a patient summary using their own clinical systems so that it can be shared or accessed by other providers to support care delivery. This project aligns the patient summary record with Canadian specifications.
