Clinical Data Available

CareConnect presents provincial and regional based data in one place to produce a consolidated patient centric view for healthcare professionals. Below is a snapshot of information available in CareConnect.

‎CareConnect displays a summary of patient information taken from the most recently updated encounter including:


  • Demographics: Most recent patient demographic data from the Provincial Enterprise Master Patient Index (EMPI). 
  • Alerts: Displays MRSA, VRE, CPO (antibiotic resistant bacteria), and Candida Auris alerts for the patient from VCHA PCIS, VPP Cerner, PHSA Cerner, NHA-Cerner and VIHA-Cerner
  • Contacts: Displays a list of next of kin, emergency contact or family member(s) of the current patient, whom the patient has indicated can be contacted
  • Family Physicians: Displays the patient's care providers
  • Languages: Displays the language(s) spoken or understood by the patient


‎CareConnect offers access to hospital encounters from five health authorities within B.C: 


  • Fraser Health
  • Island Health
  • Northern Health
  • Provincial Health Services Authority
  • Vancouver Coastal Health & Providence Health Care


‎CareConnect offers lab results from across six health authorities in B.C. as well as Community Labs (BC Biomedical, LifeLabs, and Valley Medical) and Canadian Blood Services.

 

Types of labs available include:

  • Chemistry

  • Hematology

  • Blood Bank

  • Microbiology

  • Anatomical Pathology


‎CareConnect offers access to transcribed diagnostic images and report from across six health authorities in B.C. via Provincial Diagnostic Imaging Viewer (PDIV). 


Types of reports available include:

  • Computed Radiography
  • Computed Tomography
  • Digital Radiography
  • Digital Subtraction (Angiography)
  • Mammography
  • Magnetic Resonance Imaging
  • Nuclear Medicine
  • Positron Emission Tomography
  • Radio Fluoroscopy
  • Ultrasound
  • X-Ray Angiography
For those accessing CareConnect outside of a health authority, imaging reports are viewable but not the images. Reports from community-based imaging clinics are not available in CareConnect. 

In the Documents domain, CareConnect offers access to reports and documents from six health authorities:

Content varies by Health Authority, and examples include:

  • Discharge summaries

  • Transfer summaries

‎CareConnect displays a select set of community information identified to provide the most clinical value from Vancouver Coastal Health, Fraser Health and HealthLink BC.


Examples of these documents include:

 

  • Fraser Health's PARIS Notes and Assessments (Home Health Casenote, Mental Health and Substance Use Rehab Assessment)
  • HealthLink BC's Clinical Summary from the Virtual Physician Network
  • VCH PARIS Clinical Summary (active alerts, immunization history, healthcare instructions)
  • VCH PARIS Care Plan report (client's needs, goals and clinical interventions across a broad range of disciplines)
  • VCH PARIS Notes and Assessments (Rehab Group Note, Palliative Assessment)
  • VCH Profile EMR Summary (problems, previous procedures, chronic medical conditions) 
  • BC Emergency Health Services Community Paramedicine (CP) documents (CP Visit Summary, CP Consolidated Report - if available)

‎CareConnect offers access to immunization details from the Provincial Immunization Registry (PIR), which collects data from six health authorities in B.C. and all pharmacies.  The PIR includes all Covid-19 vaccinations.  These vaccines are displayed in near real-time.


Childhood immunizations provided by community-based physicians are indirectly recorded in the PIR by the health authorities once children are school age.

‎CareConnect displays patient demographic and contact information from the following Patient Care Information Systems (PCIS):


  • Fraser Health - Meditech/PARIS
  • Interior Health - Meditech
  • Island Health - Cerner
  • Northern Health - Cerner
  • Provincial Health Services Authority
    • Children's & Women's Health Centre of British Columbia and Sechelt Hospital - Cerner
    • BC Cancer - CAIS/Cerner
  • Providence Health Care - Cerner/Eclipsys SCM
  • Vancouver Coastal Health - GE Carecast/PARIS/Cerner/McKesson STAR

Medications within CareConnect displays the Patient Medication Profile from PharmaNet, a province-wide central data system administered by the Ministry of Health that records all prescriptions dispensed to a patient at community pharmacies across B.C. The Patient Medication Profile provides a view of a patient's dispensed medications for the past 14 months. 

 

Currently, PharmaNet via CareConnect is only available for non health authority settings.
